Summary
The RN will oversee patient care through both personalized care coordination and remote monitoring of real-time health data. This includes developing and implementing care plans, monitoring patient progress, educating patients, and collaborating with providers to ensure proactive interventions and improved health outcomes.
Responsibilities
- Develop and implement individualized care plans for patients with chronic conditions.
- Monitor patient data from remote devices (e.g., blood pressure monitors, pulse oximeters, weight scales, chest sensors).
- Identify abnormal health trends and escalate concerns to physicians and care teams.
- Conduct patient assessments and follow-ups via phone, text, and email.
- Provide patient education on disease management, medication adherence, device usage, and lifestyle modifications (diet, exercise, stress management, etc.).
- Coordinate care with primary care providers, specialists, and interdisciplinary teams.
- Document patient interactions, data, and care plans in the Monovo Clinician Portal (MCP).
- Ensure compliance with HIPAA, CCM, and RPM program guidelines.
- Engage patients to maintain participation in monitoring and care programs.
- Stay current on best practices in telehealth, care coordination, and remote monitoring.
Experience
Candidates should have experience in one or more of the following areas:
- Active Registered Nurse (RN) license (multistate preferred).
- Minimum 1 year of nursing experience, preferably in ICU, Med/Surg, chronic care management, case management, or primary care.
- Experience with care coordination, patient education, and remote monitoring technology preferred.
- Strong clinical assessment and critical thinking skills with the ability to recognize and respond to urgent changes in patient conditions.
- Proficiency with EHR systems and healthcare documentation.
- Excellent communication skills, both verbal and written, with the ability to engage patients remotely.
- Self-starter, detail-oriented, and comfortable working independently in a remote environment.
- Compassionate and patient-focused, with the ability to work with diverse patient populations.

Type: Part-time, contract, permanent, full-time
Pay: $34-$40 an hour
Expected hours: 15-30 per week
Shift and schedule: Varied shifts between 6am-8pm (MST) Sunday-Saturday
Medical specialty: Critical & intensive care, primary care
